Uplifting Birth Equity Uplifting Birth Equity (UBE) is a community initiative dedicated to connecting mothers and families with the resources they need most. Our mission is to help eliminate maternal health inequities in Niagara County by partnering with local service providers to ensure families receive timely, supportive, and accessible care. UBE has organized and supports a Maternal Health Hub located within the P3 Center at Niagara Falls Memorial Medical Center, creating a central space where families can access vital services, information, and community support.

Healthy Moms/Healthy Babies Coalition of Niagara’s mission is to empower and support families in Niagara County by seamlessly connecting them with essential services and resources. Through collaboration and advocacy, we strive to cultivate a community where every parent and baby can thrive, ensuring a foundation of lifelong health and well-being for all.
Our Initiatives
The Uplifting Birth Equity Hub connects healthcare and community partners into one coordinated system of care to reduce maternal health disparities and ensure women and families have access to continuous, high-quality support before, during, and after pregnancy.
Led by Niagara Falls Memorial Medical Center in formal partnership with the Niagara County Coalition, and supported by primary care providers and the OB-GYN Department, the Hub prioritizes Black women and other populations most impacted by maternal health inequities—while serving all families in need of coordinated, compassionate care.

Our Goal
The Uplifting Birth Equity Hub is designed to create measurable improvements in maternal and family health outcomes by:
-
Increasing enrollment in primary care for pregnant and postpartum individuals
-
Improving continuity of care across clinical and community systems
-
Reducing preventable maternal complications and missed follow-up care
-
Strengthening trust and engagement between families and healthcare providers
-
Advancing health equity for populations most impacted by maternal health disparities
Who we Serve:
The Hub serves all women and families seeking coordinated maternal and primary healthcare. The program prioritizes Black women and other historically underserved populations who are disproportionately affected by maternal health disparities, limited access to primary care, and social and systemic barriers to wellness.
